摘要	目的加深对川崎病并发休克的认识,为临床早期诊断、治疗及预后评估提供参考。方法回顾性分析6例川崎病并发休克患儿的临床资料。结果 6例患儿均符合川崎病临床诊断标准,在病程中均出现收缩压低于相应年龄段正常平均血压低值20%或以上,且必须依赖血管活性药物维持。6例患儿的中位年龄6.5岁,均有一侧颈部淋巴结肿大伴触痛。出现C反应蛋白(CRP)、N末端前脑利钠肽(NT-proBNP)、心肌肌钙蛋白-I(cTn-I)、中性粒细胞比例明显增高,并伴有低白蛋白血症、低钠血症、一过性其他脏器功能损害、心电图(EEG)及超声心动图异常等。6例患儿经大剂量丙种球蛋白冲击、扩容和血管活性药物支持治疗后,均痊愈出院。结论川崎病可能并发低血压休克,尤其是具备上述危险因素的患儿更应该引起警惕。川崎病并发休克经正确及时的治疗后,一般预后较好。
其他摘要	Objective To deepen the understanding of Kawasaki disease complicated by shock syndrome and to provide information for the early diagnosis,treatment and prognosis.Methods Retrospective analysis was made of the clinical data of 6 cases of Kawasaki disease shock syndrome.Results In all of the 6 children diagnosed of Kawasaki disease,their systolic blood pressure were ≥20% lower than the low cut-off value of normal average blood pressure for their corresponding age during the clinical course,and must be supported with the use of vasoactive drugs.The median age of 6 children was 6.5 years old.They all had swollen lymph node in neck with tenderness on one side.Significant increases in C-reactive protein(CRP),N-terminal probrain natriuretic peptide(NT-proBNP),cardiac Troponin I(cTnI) and neutrophils percentage,accompanied by hypoalbuminenia,hyponatremia,temporary visceral damage,abnormal electrocardiogram and echocardiography,were also found in these 6 children.All of the 6 children were transferred to PICU,and cured after treatments with large dose of intravenous immunoglobulin,fluid resuscitation and vasoactive drugs support.Conclusions Kawasaki disease may be complicated by shock syndrome,especially for those children with the above-mentioned risk factors.Generally,the prognosis of Kawasaki disease is good after timely and proper treatments.
