摘要	目的调查危重脓毒症患者的结局,评价RIFLE标准分级和死亡率之间的关系。方法回顾性分析2003年1月～2007年7月81例行入住ICU的脓毒症患者。结果院内患者病死率48.1%。病死率和RIFLE标准分级有明显的相关性(P<0.01)。脓毒性休克,RIFLE分级和入住ICU第1天器官衰竭数量都是预测病死率的独立因素。RIFLE标准分级的严重度和器官衰竭数量、APACHE II评分及序贯器官衰竭估计(SOFA)评分有相关性。随访观查发现,出院后6个月的存活率在非急性肾损伤(non-AKI)组与RIFLE-I组,non-AKI组与RIFLE-F组,RIFLE-R组与RIFLE-F组明显不同。结论 RIFLE标准可以预测住院脓毒症患者的病死率和短期预后。
其他摘要	Objective Severe sepsis and septic shock,often complicated by acute kidney injury(AKI),are one of the most common causes of mortality in intensive care units(ICUs).This study investigates the outcomes of critically ill patients with sepsis and elucidates the association between prognosis and risk of renal failure,injury to the kidney,failure of kidney function,loss of kidney function,and end-stage renal failure(RIFLE) classification.Methods The data of 81 sepsis patients who admitted to ICU from January 2003 to June 2007 was retrospectively analyzed.Results Overall hospital mortality rate was 48.1%.Mortality rate was significantly associated with RIFLE classification(P<0.001).Septic shock,RIFLE category,and number of organ system failures on the first day of ICU admission were independent predictors of hospital mortality.The severity of RIFLE classification correlated with organ system failure number and Acute Physiology and Chronic Health Evaluation(APACHE) II and sequential organ failure assessment scores.Cumulative survival rates at 6-month follow-up after hospital discharge significantly differed between non-AKI versus RIFLE injury,non-AKI versus RIFLE failure,and RIFLE risk versus RIFLE failure.Conclusion RIFLE classification could accurately predict hospital mortality and short-term prognosis in ICU sepsis patients.
